Use these forms to suggest corrections, request an organization profile claim, or ask for help finding resources. These forms are active for partner beta submissions, but Bootstraps PA is not an emergency service.
Before you submit
Bootstraps PA uses these submissions to improve the resource guide. Please do not submit Social Security numbers, medical records, court documents, financial account numbers, passwords, confidential shelter locations, or detailed facts about abuse, medical care, legal disputes, or immigration status.
Use this for changed phone numbers, hours, eligibility, services, intake steps, addresses, websites, or closed programs.
Use this if you work for a listed organization and want to help confirm or maintain the listing.
Use this for general navigation requests. Response time is not guaranteed, and the form is not monitored for emergencies.
